Permission to Skate/NRP Passports
Players may request a Permission to Skate (AAA) or a Non Residential Passport (AA) by completing the online form provided on our website.
Before submitting your request for Permission to Skate, please review the following:
1) At this time we will only be processing the PTS for tryouts that are being held in the Spring. For Tryouts that are being held in the Fall please apply for your PTS in August.
2)Tryout time is a busy time of the season administrative wise, so please be patient and refrain from sending multiple emails asking for your PTS.
4)The PTS form will not be made available from OMHA before April 15th, 2023.
ONLY SUBMIT the FORM ONCE. As Soon as PTS forms are made available you will receive your form by email. DO NOT SEND MULTIPLE EMAILS ASKING FOR YOUR PTS FORM
5) PTS Forms and NRP Passports are available to QWMHA residentially qualified players only. If you played here as an NRP or in the house league, you must request this documentation from your home centre.
6) Players requesting a PTS form or NRP Passport must complete the online form available on our website. Emails will not be considered as a proper request.
7) Players requesting a reverse NRP to a centre with a lower base category of QMHA (U12 and above) must try out for and be
released
from their home centre team first.
8) Players must be in good standing with both QWMHA and their respective teams. Forms will not be released to anyone that owes funds.
