Please complete the form below to request a reimbursement from Quinte West Minor Hockey for expenses related to becoming certified for a Quinte West Minor Hockey team. When requesting a refund please note the following:

1. All requests are subject to approval and must be submitted through this website. The request must include all of the information requested below (any incomplete applications will not be processed).

2. Quinte West Minor Hockey will only reimburse expenses related to the current season (the team official must be on a QWMHA approved team roster).

3. All receipts must be attached for the refund to be processed.

Please allow six weeks to process the request.  Please note that until the team roster is approved, reimbursements cannot be processed.
