QWMH had decided to provide registration in blocks as we phase into a full return of regular programming. Members interested in playing in block 1 must register with both the Hockey Canada registration and this form to be considered.

Tier Level Request

If numbers warrant we will tier our house league into two tiers. Tier 1 - Tier 1 is geared towards the player that has experience playing competitive hockey. Players requesting to play in this tier must be approved to play. Space is limited and approved players will be contacted for payment. Tier 2 - Tier 2 is geared toward players who have experience playing at the house league level. Space is limited in this tier and approved players will be contacted for payment.
